Vascular Trauma in Extremity Fractures, Management and Prognosis: A Mini review

Abstract
Fractures remain a significant health problem globally, and their incidence has increased by more than 30% since the 1990s. Furthermore, fractures associated with vascular injury are not uncommon. A vascular injury may be classified as penetrating, or Penetrating blunt injury is often obvious, resulting in laceration of the vessel wall. Meanwhile, blunt trauma cause variable damage to the vessel wall, from miniscule intimal flap to transmural damage with extravasation or thrombosis. There are several methods to diagnose vascular trauma in limb fractures. However, physical examination is still the first and foremost modality in diagnosing vascular trauma. Then, we can use the Ankle-brachial index (ABI) test or Color Doppler ultrasound, and angiography is required if ABI is <0.9 for established diagnosis. CT angiography (CTA) can also be a valuable tool in assessing the peripheral vascular injury.
On the other hand, tools for diagnosis and the management of peripheral vascular injury have evolved in recent years. There is various management of vascular trauma, such as Non-operative management, Endovascular management, Intravascular shunt, End-to-end anastomoses, and Interposition Graft. This all management depends on the grading and condition of the wound. The principles goals of treatment are control of the bleeding and restoration of distal perfusion to achieve limb salvage. There are several factors affecting the outcome of peripheral vascular injury. Several worse predictors are high energy mechanism, blunt trauma, and associated musculoskeletal injuries. Blunt trauma and musculoskeletal injuries are predictors of a higher amputation rate, worse functional recovery, and increased length of stay.
